1. Have these ready to upload:
   • Birth Certificate
   • Immunization Record
   • Most recent Report Card
   • Most recent Standardized Testing Report
   • Baptismal Certificate (if applicable)
   • Medical Forms found here.
2. Click the Application button above and follow the link to create a PowerSchool Enrollment account.
3. Fill out the online form, upload documents, and submit. PowerSchool Enrollment lets you “snap” shared information if you register more than one child.
4. After submission, our school will contact you with further instructions.

New Students on a Trial Basis
All new students are accepted on a trial basis for the first two months of enrollment. If severe disciplinary problems occur, or if a student is unable to keep up with class assignments and maintain passing grades, the school may review with parents or guardians whether the child should continue at Saint John the Apostle School.

Immunization Requirements
Saint John the Apostle School follows the NJ State School Immunization Requirements. In accordance with the Archdiocese of Newark, no religious exemptions are allowed. Applicants whose immunization records are not complete will not be accepted.

1. They do not wear diapers or pull-ups at school.
2. They can inform the teacher when they need to use the bathroom.
3. They can manage their own hygiene with minimal assistance.       St. John the Apostle School reserves the right to request that a student be withdrawn if they are unable to function independently in the restroom without frequent accidents or incidents.
